Jacqueline J. Cook, age 77, of West Bend, passed away on Sunday, May 5, 2013 at her residence.
She was born on June 9, 1935 in Milwaukee to the late Harvey and Erma (nee Paluzzi) Cook. Jackie graduated from West Bend High School in the class of 1953. She worked as a Phlebotomist at the Blood Center of Wisconsin, the West Bend Mutual Insurance Company through the Threshold, and as a Massage Therapist.
Jackie is survived by her daughter, Charlene Chapman of Wauwatosa; five grandchildren, Kelsey Chapman, Eric Chapman, Alec Fantl, Kathryn Fantl, and Jacob Fantl; daughter-in-law, Eda Fantl of Delafield; three sisters, Jeanne Mueller of West Bend, Brenda (Ron) Knoeck of Beechwood, and Dorothy (Don) Brath of West Bend; sister-in-law, Diane Cook of McHenry, IL; other relatives and friends.
She was preceded in death by her parents; son, Kenny Fantl; father of Kenny and Charlene, Peter Fantl; brother, John Cook; and two brothers-in law, LeRoy Mueller and Paul Thompson.
A funeral service in remembrance of Jackie will be held on Friday, May 10th at 6:00 pm at the Myrhum-Patten Funeral Home, 1315 W. Washington Street, West Bend with Rev. Francis Baiocchi officiating. Visitation will be at the funeral home on Friday from 3:00 pm until the time of service. Inurnment of Jackie's cremated remains will be on Saturday, May 11th at 11:00 am in Washington County Memorial Park in West Bend.
